Our promise to you is to provide personalized and quality dental care as well as exceptional customer service.

Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to Dr. Swanson and our team.

During your first appointment we will review your medical/dental history, complete a comprehensive exam, preventive x-rays (if needed), periodontal and oral cancer screening and a cleaning. Depending on your needs additional follow up may be needed.  

Our goal, by providing excellent dental care, is to help you feel and look your best.

Please bring the following items to every appointment:

• Insurance Card (if applicable)
• Photo ID (18 years of age and above)
• Up to date medication list (if applicable)

Patient Forms

We kindly ask that you complete your New Patient Forms in advance of your office visit. This will allow us to dedicate the entire appointment to your dental care.

Once in a while we will request that current patients complete updated patient forms. This helps us to ensure we have current information for all of our patients. If you have been asked to complete new forms we kindly ask that you complete them prior to your appointment.
